Registering at the Practice

If you live within our Practice Boundary, and are eligible to register for NHS services, you are welcome to join Nethergreen Surgery as a patient.

If you wish to register as a new patient with us, you will need to call at the Practice to complete a few formalities; however, in order to speed up the process when you call, you can download the forms and complete them at home.

You will  need to complete a registration form that can be downloaded here, if you don't have your NHS Medical Card.

You will also need to complete an Ethnicity form.

 Finally, click here  for a map of the Nethergreen catchment area, to see if you live within the boundary. Please be aware that if you move out of our area you will be expected to re-register at a Surgery whose area covers your new address.
